The Guelph and Area Community has been approved to be one of Ontario's first Ontario Health Teams. As such we are responsible for transforming care for community members whose complex needs related to mental health, addictions, and/or homelessness result in heightened vulnerability. To achieve the goals of reducing reliance on crisis services, providing better quality care for clients and a better client experience, the Guelph CHC, in partnership with several organizations is building an inter-disciplinary health clinic that will operate on a same day/walk-in model. Clients coming to this clinic will have access to a team of primary care providers, psychiatry, nurses, social workers, outreach workers, peers and others who will work together as an integrated team to provide wrap-around care for clients and will be empowered to find creative solutions to quickly and effectively meet client needs.

Client Centred Medical Care

  • Provides primary care services, including comprehensive assessment and treatment, supportive client counseling, crisis intervention, prenatal and neonatal care.
  • Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews, and discussion of problem cases and answering client inquiries
  • Refers to specialists, hospitals, or other agencies as required.
  • Work within a determinants of health framework which emphasizes health promotion and education, disease prevention and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients
  • Advocates for patients with other medical workers, agencies, or institutions
  • Works as a member of an inter-disciplinary team, which includes supporting team members as appropriate, or assigned, including clinical consulting with Nurse Practitioners and other Clinical team members.
  • Participate in the Centre's on call service as directed (consisting of courtesy hospital visits and phone calls in rotation with other physician colleagues) and occasional mobile visits as necessary
  • Assures standards of patient care are maintained in accordance with accepted medical standards & GCHC policy.

Outreach & Community Development

  • Participates in other GCHC programs/projects, as appropriate.
  • Represents GCHC in community projects or agency activities in which they become involved.
  • Advocates on health & related matters, on a larger front, for the community as a whole.

Primary Care Education

  • Along with other members of the Clinical Team, supervises & evaluates medical & nursing students at GCHC.
  • Acts as a medical resource to other GCHC staff &/or Board members, either individually or through representation on committees.
  • Maintain professional development through continuing education, including Practice Based Learning opportunities.


  • Participates in routine program, team, & general staff meetings, & in Board or committee meetings, as appropriate/required.
  • Participates in data collection, analysis, & ongoing monitoring/evaluation regarding GCHC programs & activities for various purposes (e.g. program planning, research, etc.).


  • Licensed Family Physician, as per the requirements of the Ontario College of Physicians and Surgeons, with CCFP certification
  • Possess or is eligible to obtain required malpractice coverage with CMPA (this coverage is eligible for reimbursement)
  • Experience working with communities and individuals who face barriers accessing traditional health care institutions (such as individuals facing poverty, language barriers, discriminations, mental health and substance use issues, etc.)
  • Previous experience in community health/social services and/or a values based organization is an asset
  • Excellent communications skills, both verbal and written, along with the ability to prioritize and multi-task
  • Proficient in the use of computers and various other software applications including Word, Outlook, and electronic health records
  • Ability to work interdependently and respectfully in an interdisciplinary team environment
  • Proficiency in other languages is an asset
